Omegas aren't very common, all of them are considered a national property and don't have many rights. Sherlock's parents tried their best to hide their son's secondary gender, but something went wrong, maybe someone betrayed their trust (a doctor? Someone who works as a servant for them? A "friend" of the family?) and reported that they'd been hiding an Omega. So Sherlock was taken away.

Omegas can be bought from the government by rich citizens or given as a reward.

Sherlock has been kept in some sort of facility for Omegas for a long time. Maybe he was bought by someone in the past when he was young, but he was later returned and is considered damaged goods now (maybe he was actually damaged pretty badly by his previous owner and has scars, and maybe Sherlock himself had damaged his reproductive system with heavy drugs just to avoid being impregnated against his will or to cause miscarriage). And being damaged goods and more than 35 y.o. with a bad temper isn't an Omega rich people want to waste their money on.

Still, those who own Omega facilities want to get rid of him, because free people don't pay taxes to feed and cloth an unwanted Omega. So he gets foisted on John Watson, the recently invalided army captain. John is surprised since, while he may be some sort of a war hero, Omegas are usually given to people in exceptional cases. He doubts he wants an Omega at all, having too many problems of his own. Also, he realises that Sherlock isn't a fresh, young Omega people usually buy so he sort of sees what's going on.

I would love things to work out for them eventually and them finding comfort and happiness with each other.
