Sherlock and John are in the early phases of a relationship where they've kissed and touched plenty, but haven't had sex yet or really discussed what this 'thing' between them is let alone discuss kinks.
Sherlock and Irene are friends*, but she knows he's serious about John and respects that. John dislikes Irene, and chooses to deal with this by trying to ignore the fact that she exists.
Irene figures out that Sherlock has a latent interest in crossdressing and that John would be very receptive to it, so she teaches him how to do it.
Meanwhile, John isn't pleased that Sherlock has been spending more time with Irene again lately, and--is that her underwear on his bedroom floor?! Misunderstandings and angst ahoy, since Sherlock's not ready to admit that actually those are his. Irene may or may not pick up on John's jealousy and intentionally feed it for the purpose of glorious trolling.
Of course, it all get resolved in the end (preferably with porn) and it's something they can look back on and laugh about. John may not exactly like Irene by the end, but he's no longer bitter about her friendship with Sherlock.
*The way I see it is that there was mutual attraction between the two of them that eventually settled into a friendship where they alternately troll the hell out of each other and help each other out of tricky situations. The only Sherlock/Irene in this should be all in John's imagination.
