Sherlock trips and falls into a relationship with a charming man who appears patient and indulgent in Sherlock's weird acts. But over time (weeks), Sherlock comes to realise that he's being manipulated into sexual favours and has been raped for months. Sherlock tried to break up and as punishment for it, the boyfriend locks him in a cage and refuses him release. He edges Sherlock sometimes or completely ignores him. Sherlock for once cannot save himself, he hates the thing caging his poor penis. His entire groin hurts. It's tender and the psychological and sexual abuse has made Sherlock even more repulsed at touching his own genitals. The cage allows Sherlock to clean himself but doesn't let him orgasm and makes it difficult to get hard.

Sherlock escapes back to 221B and John is somewhat relieved because he has his friend back. Over months, Sherlock mental state gets worse, his genitals are swollen painfully and he has started walking carefully and become over protective of his groin area as the gentlest bump hurts.

Then John sets to removing the device (he's a doctor, he's seen this sort of thing before, but on willing people).
