Sherlock is in an unhealthy relationship. His Dom forced him into subdrop and is punishing him by not helping him out. He's been in sub drop for months now. Sherlock is out shopping for groceries alone, he's terrified and suffering and obviously malnourished. He's in his own world, it's a miracle he's even outside. But it doesn't stay that way for long. He faints and is taken to hospital where John treats him.
Sherlock falls into a coma for maybe a couple of days and when he wakes, he is panicked. They've not called his Dom because there's no ID on Sherlock so they don't know who he is, even if they did, the extended abuse is obvious they call authorities instead. They've called some service who helps victims of domestic abuse. Sherlock is still suffering and he can't really comprehend anything. He just wants his Dom even if he is terrified. They tell him that he needs to build his strength before he can have any visitors. It becomes apparent that Sherlock is still in subdrop and everyone is completely shocked. Sherlock refuses food and water and is in a constant state of anxiety. So they've got him hooked to IVs and other things. A therapist is called and eventually when they prod Sherlock enough about why he is refusing food, Sherlock admits that he isn't allowed food. That his master is punishing him so he's not allowed to eat.
John decides that's it and he's calling the authorities, Sherlock isn't going anywhere near his abuser. While under John's care, Sherlock's dom is arrested and thrown in jail and with no where to go, Sherlock moves in with john after he is discharged and so starts the long road to recovery. And it is tough especially when john leaves for a week long conference and comes home to a Sherlock who's fainted from lack of food... Because "you didn't say i was allowed to eat".
