Sherlock is a hoarder. Mycroft sweeps through every once in a while and has his people throw out pretty much everything in the flat, which only makes Sherlock more desperate to hoard everything he can get immediately afterward. Maybe this has contributed to Sherlock getting kicked out of flats in the past, and/or is related to his drug use?
Anyway, John comes along, and all of a sudden 221B is under control. Not clean, never actually clean, but John refuses to let Sherlock accumulate intestines and toes and microscope slides and fungal cultures and it's at least livable. But then Sherlock fakes his death and John goes off and gets married and Sherlock's living on his own again in 221B after he gets back and everything goes to hell in a handbasket.
I'd love to just see someone's take on this - how it bears on Sherlock and Mycroft's relationship, how John reacts when he finds out how serious Sherlock's problem is, whether Sherlock gets a handle on his hoarding or not, etc.
